Calvary Memorial Church, 2025–2026



Calvary Awana Clubs are open to children 3* years old through 5th** grade. We welcome children from all churches and backgrounds.

*We ask that children be at least 3 and potty-trained by the start of the club year to attend our preschool group, Cubbies.

**Some 6th graders may wish to continue in T&T an additional year to earn their 4th book award. Otherwise, we recommend that 6th graders step up to youth group.

We love to know where our Awana families are coming from and how we can support you within the global church body. It also helps us make connections with and between the clubbers.

Liability Waiver & Medical Emergency Release

I/we agree to release and hold Calvary Memorial Church and its agents harmless from any liability, actions, causes of actions, claims, expenses, demands and damages because of injury to my/our child(ren) or property which arise related to events or activity participation. I/we expressly warrant that the child(ren) registered are capable of withstanding both the physical and mental demands of the planned activities, and I/we expressly assume all risks, known or unknown. In case of an emergency, I/we give permission for our child(ren) to receive medical treatment after reasonable effort has been made to reach parents/ guardian by phone at the numbers listed in our registration. I/we assume the responsibility for any cost connected with such treatment and hereby release Calvary Memorial Church from any liability thereof. Should it be necessary for my child or youth to return home due to medical reasons, disciplinary action, or otherwise, the undersigned shall assume all transportation costs and responsibility. 



Transportation Consent

I/we give permission for this child to ride a bus or other vehicle approved and operated by a licensed adult, to the annual Christmas caroling event at Lake Minnetonka Shores Senior Living in December 2025. My child(ren) and I understand that SEAT BELTS MUST BE WORN AT ALL TIMES when available during transportation. I/we understand that care will be taken to ensure the health, safety, and welfare of my child. Though safety is top priority, I/we will not hold Calvary Memorial Church or its volunteers or agents liable for any damages or harm resulting from vehicle operation or otherwise during the off-site event. I/we realize and accept that in the event my child’s behavior is adversely affecting the safety of the vehicle or otherwise, the organizers will notify parents and arrange to return my child home.



Photo Use

I/we give permission allowing photos of our family/children to be occasionally used in church publications, including but not limited to slide shows, social media, and website.
