Personal Information Use


For the safety of our child participants, we will ask you for contact and personal information to verify your eligibility to volunteer in our VBS program, and in case of emergency.

According to Calvary's child protection policy, all children's ministry workers ages 18+ must pass a background check (if not on file). This is being offered at no charge to you.

Your privacy is important to us. We will never share this information with third parties.

Volunteer Preferences


Liability Waiver & Medical Emergency Release

I agree to release and hold Calvary Memorial Church and its agents harmless from any liability, actions, causes of actions, claims, expenses, demands and damages because of personal injury or property damage which arise related to events or activity participation. I expressly warrant that I am capable of withstanding both the physical and mental demands of the planned activities, and I expressly assume all risks, known or unknown. In case of an emergency, I agree to receive medical treatment after reasonable effort has been made to reach an emergency contact by phone at the number listed in my registration. I assume the responsibility for any cost connected with such treatment and hereby release Calvary Memorial Church from any liability thereof.


Photo Use

I/we give permission allowing photos of myself to be occasionally used in church publications, including but not limited to slide shows, social media, and website. 

Consent to Background Check


For volunteers who have had a background check in the last 3 years, you will not be required to be screened again.

For all others, please be on the lookout for an email from Checkr Background Service to begin your screening.


Statement of Faith

It is important to us that all our volunteers are rooted in biblical principles so that we can guide children toward the knowledge of Christ in unity.

WE BELIEVE in the Scriptures of the Old and New Testaments as verbally inspired of God, inerrant in the original writings, and that they are the supreme, complete, and final authority in faith and life.
WE BELIEVE in one God, eternally existing in three persons—Father, Son, and Holy Spirit.
WE BELIEVE that Jesus Christ was begotten by the Holy Spirit and born of the Virgin Mary and is true God and true man, with a perfect and sinless humanity.
WE BELIEVE that man was created in the images of God, that he sinned by disobedience, and thereby incurred not only physical death but also that spiritual death which is separation from God. We further believe that all human beings are born with a sinful nature and are sinners in thought, word, and deed, not of constraint but by choice.
WE BELIEVE that the Lord Jesus Christ died for our sins according to the scriptures as representative and substitutionary sacrifice, and that all who believe in Him are justified on the ground of His shed blood.
WE BELIEVE in the resurrection of the crucified body of our Lord, in His ascension into heaven, and in His present life there for us as High Priest and Advocate.
WE BELIEVE in "that blessed hope," the personal and the imminent return of our Lord Jesus Christ.
WE BELIEVE that all who received by faith in the Lord Jesus Christ as personal Savior are born again through the Word, by the Holy Spirit, and thereby become children of God.
WE BELIEVE in the bodily resurrection of the just and the unjust, the everlasting blessedness of the saved, and the everlasting, conscious punishment of the lost.
WE BELIEVE in the personal existence of Satan, the father of lies, a murderer from the beginning, the false accuser, the great deceiver of the nations. We believe that his power was broken at the Cross of Calvary and that he will be finally committed into the lake of fire with all his angels to be tormented forever and ever.
